Services Agreement

This Services Agreement (the "Agreement") is made as of , by and between TeamFicient , an Illinois limited liability company, having its principal

place of business at 2058 North Western Avenue, Chicago, Illinois 60647 (the "Provider"), and ,having its principal place of business at (the "Client") (collectively hereinafter referred to as the "Parties" and individually referred to as the "Party").

WHEREAS, the Client desires to contract with Provider to supply remote personnel to support Client's business in accordance with the terms of this Agreement.

WHEREAS, Provider is in the business of providing remote professional services including but not limited to administrative assistance, bookkeeping, sales and customer support and IT assistance, to its clients (the “Business”) and has agreed to provide consulting services to the Client, based on Client's petition.

NOW, THEREFORE, in consideration of the above recitals, which are hereby incorporated herein, and in consideration of the mutual promises made herein, the receipt and sufficiency of which are hereby acknowledged, the Client and Provider further agree as follows.
